Services Severe & Complex Mental Health needs

Program Plus Social - Delivered by Primary & Community Care
Services From 1 July 2018 a new service commenced on the Gold Coast for people with severe mental illness. This service is called the Plus Social program.

The Plus Social program provides:
 
  • Service information, intake and triage service into currently funded Gold Coast Primary Health Network (GCPHN) organisations
  • Clinical care coordination, and
  • After hours community-based space (known as The Hub).
 The clinical care coordination component provides:
 
  • supports for individuals with, or for young people at risk of, severe mental illness
  • time limited (up to 26 weeks) clinical care coordination and wellbeing program
  • structured, recovery and goal oriented services focused on creating significant improvements in quality of life, health and wellbeing
  • a patient-centred approach and a healthcare neighbourhood model, to support integrated care and enhanced community access to supports.
Referrals Referrals to the Plus Social program can only be from a GP or private psychiatrist.
Contact Details The after hours drop-in service at The Hub in Mermaid Beach is open 6pm-10pm weekdays and 11am-10pm weekends for new and existing clients of Plus Social. Address: 2580 Gold Coast Highway, Mermaid Beach QLD 4218.

Program Partners in Recovery (transitioning to NDIS)
Services Partners in Recovery (PIR) facilitates better coordination of and more streamlined access to the clinical and other services and support needs of people experiencing severe and persistent mental illness with complex needs requiring a multi-agency approach.

The PIR initiative is transitioning to the National Disability Insurance Scheme (NDIS) on 30th June 2019.

The transition from PIR to NDIS
Partners in Recovery (PIR) is in the process of transitioning participants to NDIS. This will be occurring as each Local government area is transitioned into the NDIS. NDIS in Queensland.

PIR-Gold Coast will be transitioning eligible consumers to the NDIS from 2nd January 2018, in preparation for the NDIS roll out on 1st July 2018.

For those who are not eligible for NDIS or who choose not to enter the scheme PIR will continue to provide support.
