Returning BreastScreen Queensland Clinical Examination Outcome Forms


BreastScreen Queensland (BSQ) are advising general practitioners that completed Clinical Examination Outcome Forms should be returned to BSQ with a recommendation (i.e. routine rescreen, further investigation required, etc). Promptly returning these forms greatly assists the local BSQ Gold Coast Service to meet BreastScreen Australia National Accreditation Standards. 

Where a BreastScreen assessment is requested, the area of concern and reason need to be indicated. If diagnostic investigation is sought, please specify where the patient has been referred to.
