Gold Coast Primary Health Network (GCPHN) and Gold Coast Health are seeking Residential Aged Care Facilities (RACFs) who would like to participate in an innovative program aiming to enhance palliative and end of life care for residents.
The program will help RACFs to meet the 2019 Aged Care Quality Standards by providing support and training in palliative and of life care (including advance care planning) and facilitating partnerships between aged care, general practice and Specialist Palliative Care services. The focus is on building a sustainable, palliative and end of life care capability in participating RACFs.
RACF staff will have the opportunity to receive tailored education and be mentored by a Specialist Palliative Care Clinical Nurse Educator. The program aims to work closely with general practitioners working in RACFs.
If you are a general practitioner working in RACFs, GCPHN and Gold Coast Health are keen to work closely with you to increase knowledge and capability on palliative care.
The opportunity can only be offered to five Residential Aged Care Facilities (RACFs) and is expected to commence in January 2020.
