GCPHN Publication Generally Speaking for Gold Coast general practices

In the December edition

Supporting mental health for Gold Coast aged care residents

GPs who service residential aged care facilities (RACFs) on the Gold Coast now have the option to refer patients into a new program to access psychological services while living in residential aged care. Change Futures has been commissioned to provide psychological support for residents with mild and moderate mental health conditions, through one-on-one and group interventions.

Recognised as a national priority by the Federal Department of Health, funding has been made available to Gold Coast Primary Health Network to implement new services to address a gap in services and complement existing services such as Older Persons Mental Health and dementia support services.

Change Futures is currently supporting 25 facilities across the region. Facilities can request to join the program and referrals are welcomed. It is planned that all RACFs on the Gold Coast will be part of the program within two years. To refer into the program phone: 07 5648 0424.

Wishing you, your teams and family a Merry Christmas and every success for 2020.

We would like to take this opportunity to thank you and your teams for the contribution you have made to the community of the Gold Coast over the last year. We value your contribution to the primary health sector and are committed to working with you in 2020 to continue to build an integrated high performing service sector on the Gold Coast. From the team at Gold Coast Primary Health Network.

This is not only in the clinical space (and particularly mental health activities with our partners), but also in the overall improvement in links across the health sphere.

Reflections of a Clinical Lead

Collaboration - Cliché or cornerstone of General Practice?

‘Collaboration’ has been promoted as an approach for improving health care so often that it risks becoming a cliché. Its proponents believe collaboration increases efficiency, enhances patient satisfaction and produces high-quality services. However, the collaboration experiences of many patients and general practices often fall short of expectation.

The reason is rarely collaboration itself, but rather ‘why’ and ‘how’ we choose to collaborate. Find out more

Sharing Knowledge about Immunisation (SKAI)

The Sharing Knowledge about Immunisation (SKAI) website has developed an eLearning module and website for healthcare workers. The SKAI eLearning module is designed to help you adapt your clinical communication skills to meet the needs of all parents, whether they are ready to vaccinate, have questions or intend to decline vaccination altogether. The module is accredited for CPD points and also provides an introduction to the SKAI website for providers.

The SKAI website for providers contains information to help you recognise parents who can benefit from your time and expertise (and avoid extended or confrontational interactions). It also provides answers to parents’ common questions and concerns. You can access the modules here.

Remember to keep us updated on the status of your PIP QI registration.

If you are a newly registered practice or are no longer participating in the PIP QI Incentive, it’s important to let GCPHN know. We require this information to advise the Department of Health that your practice data submission has been submitted to meet PIP QI requirements and ensure that you receive the correct incentive payment. You can update your information by contacting our Practice Support team on practicesupport@gcphn.com.au or 5612 5408.

Understand the ‘continuous quality improvement’ requirements for PIP QI

One of the eligibility requirements for the PIP QI incentive is that practices ‘participate in continuous quality improvement (CQI) in collaboration with their PHN.’ Your CQI activities can be around the PIP QI 10 Improvement Measures or an area that reflects the needs of your practice population. You do not need to submit evidence of your CQI to GCPHN or the Department of Health, however you will need to keep evidence of these activities for at least 6 years. Our PIP QI Incentive webpage has examples, templates and guides you can use for your CQI activities.

GCPHN Offers CQI training and support

Gold Coast Primary Health Network offers regular Data Extraction Tool and CQI training for general practice staff. These training events will provide an overview of data extraction tools available, how to use these tools to support your CQI activities and a facilitated workshop to assist with potential CQI activities for your practice. If you are interested in attending one of these training sessions, please contact our Practice Support team on practicesupport@gcphn.com.au

10 Christmas eating tips to assist you this festive season

Written by Amanda Clark, Advanced Accredited Practising Dietitian

Christmas is a time of food, family and festivities.  We eat drink and be merry but this can sometimes come at a price. Evidence shows that around half of the weight gained each year occurs over the Christmas period from the last week in November to the first week in January.  Here’s some great ideas for healthy Christmas fare whilst skipping this year’s dose of extra weight.

  1. Stock your fridge and pantry with healthy choices and a small amount of traditional Christmas add-ons. Think cherries, mangoes, cashew nuts, protein balls and a single serve pudding or Christmas cake.
  2. Cater for the day, not the month. If there is 10 people coming for Christmas day, prepare food for 10, not 50!
  3. Plan for the party. If there is a party that you know will be deep fried nibblies, go in satisfied. Eat a small healthy lunch before attending so you can be more discerning about your intake.
  4. Don’t be a food pusher. If you’re offering around amazing homemade treats, don’t be offended by a polite “No thanks”.  It’s no reflection on you.
