End of Life Directions for Aged Care (ELDAC) is a free, government-funded resource designed to connect you with evidence-based tools and resources in palliative care and advance care planning.
The ELDAC Home Care toolkit has been developed by the University of Technology Sydney with palliative and aged care experts to assist staff working in aged care services supporting client’s at home to provide information and resources on providing palliative care and advance care planning. In addition to clinical information the toolkit includes a personal learning assessment and organisational resources to embed palliative care and advance care planning within your service. There are links to tools, fact sheets, guides, discussion starters, family resources and e-learning modules.
