Kalwun Aged Care Service - Home & Community Care
The Home and Community Care program is a joint Commonwealth, state and territory initiative. It funds basic maintenance and support services to help frail older people and people with a disability to continue living in their own community.
Kalwun Home and Community Care (HACC) is designed to support eligible persons to remain living in their own homes in a way that promotes independence and reduces their risk of premature and/or inappropriate admission to residential aged care.
The service aims to assist frail older people and younger people with moderate, severe or profound disabilities and their carers. Kalwun HACC provides basic support and involves an assessment process. Services can include help at home and help in getting out into the community.
Kalwun HACC services provided in the community may include:
Centre-based day respite
Social and Transport support
Allied health services
Kalwun HACC services provided at home may include:
Domestic assistance – Household jobs like cleaning.
Personal care – Includes help with bathing or showering and hair care.
Home maintenance – Includes care of a client’s lawn and annual spring cleans.
Kalwun HACC also provides food services, these services may include:
providing meals at a community centre or day centre
helping with shopping for food, preparing and storing it
delivering meals to your home.
In order to assess your needs, a person from a Kalwun HACC will talk to clients about their daily living. The information provided is written into the HACC Ongoing Needs Identification (ONI) form. This is used by the all HACC agency to decide whether people are eligible for a service and what type of service they need.
