Adult Community Health Service (Queensland Health) Chronic Disease Wellness Program - Helensvale

The Chronic Disease Wellness Program aims to teach you how to stay well, prevent avoidable admissions to hospital and support you to improve your quality of life living with a chronic health condition. Our community based specialist multidisciplinary team will partner with you to achieve your goals.


  1. Heart Failure
  2. Chronic Obstructive Pulmonary Disease (COPD)
  3. Chronic Kidney Disease (CKD)
  4. Type 2 Diabetes or Pre Diabetes
Exception: Indigenous Chronic Disease services target community members with risk factors or concerns relating to chronic health problems.

Your GP remains your primary care provider at all times during the program and receives correspondence about your progress.

The Program includes:
  1. Physician/multidisciplinary clinics
  2. Individual and group education about the specific disease
  3. Prescribed exercise programs for clients with Heart Failure and COPD (Pulmonary Rehabilitation)
  4. Individual multidisciplinary assessment as required (Specialist Nursing, Pharmacy, Physiotherapy, Exercise Physiology, Social Work, Psychology, Occupational Therapy, Dietetics, Podiatry, Speech Pathology supported by Indigenous Health Workers and Allied Health Assistants)
  5. Access to groupwork and clinics eg smoking cessation, behavioural weight management (Appetite4Change), Mindfulness for Mood Enhancement, education Advanced Health Directives
  6. Indigenous Chronic Disease services: provided at Community Health Centres or in partnership with Aboriginal and Torres Strait Islander Medical Centre. DO NOT REQUIRE A DIAGNOSIS.
    1. Indigenous Heart Health Clinic: a screening and early intervention cardiac Screening clinic that is facilitated by health workers and includes Cardiologist, Cardiac Scientist, Clinical Nurses and Dietitian.
    2. Mungulli Community Education: education about chronic disease for the Indigenous community,  provided in Indigenous gatherings in a yarning style
    3. Mungulli Wellness Clinic: clients with concerns about chronic disease participate in a Flinders Care Plan which negotiates motivational goals. Indigenous specific clinics deliver culturally safe access to respiratory, diabetes, CKD multidisciplinary assessment, care and supported referral. Case conferencing with GP available
The service operates out of Helensvale Community Health, Robina Health Precinct and Palm Beach Community Health Centre. All referrals are via the Central Intake Unit Ph1300 668 936 Fax 1300 668 536. GP can access referral templates on (templates outline required investigations). GPs can discuss referrals with specialist nurses by telephoning the Central Intake Unit and requesting the relevant specialist nurse.
