Chronic Disease Wellness Program- Community based (Gold Coast Health)

Adult service offering intensive and specialised multidisciplinary care for clients with complex care need related to Heart Failure, Respiratory, Diabetes, Chronic Kidney Disease. Care is also coordinated across these service areas for clients with multiple chronic conditions.

The service:

  • works with GPs and acute services to improve quality of life and reduce risk of hospital admission. GP Referral Templates available on this site: See 'Referral' (Adult Community Health)
  • supports development of proactive self management skills. Programs are time limited usually up to 12 weeks.
Components of care:
  • Case manager allocated as single point of contact
  • Comprehensive multidisciplinary assessment and care planning
  • Advance Care Planning
  • Physician and multidisciplinary clinics
  • Centre and home based services based upon need
  • Group and individual disease specific education
  • Customised exercise programs: Heart Failure, Pulmonary Rehabilitation
  • Medication safety
  • Smoking cessation (offered only as part of the program- no external referrals)
  • Partner support
  • Group sessions:
    • Mindfulness 4 Mood Enhancement (targeting anxiety and depression                               
    • Appetite 4 Change (Behavioural Weight Management)       
  • Linkages with ongoing maintenance options
