Lives Lived Well Drug & Alcohol Transition Support

Transitioning in and out of drug and alcohol residential rehabilitation are big steps. We work with you to make it as easy as possible and establishing strong foundations as you move into a new chapter.

Personal and flexible care

Our services can be delivered in the way that best suits you, such as  telephone, face to face, individual or in groups. Our team of drug and alcohol caseworkers bring  cultural knowledge and respect to our services.

Before starting

Before starting a live-in program, we can help you prepare for the change ahead and start to build helpful skills. We will also work with you to form personal strategies and practices to ensure that you are in the best possible position to begin. This includes:

  • assessment and screening for services
  • information about safe use and harm reduction
  • options for interim support: individual
  • counselling, group programs or outreach
  • access and referrals to external supports
After finishing
Once you’ve completed a live-in program, the help available continues with your recovery in the  community. This begins with the availability of supported accommodation and extends into ongoing counselling and further skill development. Our team can work with you on things like:
  • relapse prevention
  • budgeting
  • relationships and parenting
  • education and employment
