DESMOND - Diabetes Program
Diabetes Education and Self-Management for Ongoing and Newly Diagnosed (DESMOND) is the only evidence based self-management education program for people with type 2 diabetes in Australia.
DESMOND aims to help participants become more confident in managing their own condition. The program assists the person living with diabetes to achieve sustained weight loss, increased physical activity levels, and improved well-being. DESMOND originated from the UK where it has been an integral part of the type 2 diabetes standardised care pathway since 2003. Benefits of the program are widely documented.
Funded by the National Diabetes Services Scheme (NDSS), DESMOND offers an additional service to complement the existing suite of programs and resources delivered by Diabetes Queensland. Continuing Diabetes Queensland’s commitment of providing quality education for people living with type 2 diabetes, DESMOND has now replaced the previous group program - Diabetes - What now?
What does the program involve?
DESMOND focuses on providing people with type 2 diabetes practical skills for managing their condition. The program recognises that there is no ‘one size fits all’ approach to diabetes management. It is intended to provide a welcoming and non-judgmental space where people living with type 2 diabetes can plan how to manage their condition.
DESMOND is a six hour program, delivered as either a full day or two half days session. The program is built around group activities, with participants encouraged (not coerced) to share experiences and talk about what type 2 diabetes means to them. Within the session participants create their own health plans, based on their own needs and circumstances.

Sessions can take up to 10 participants living with diabetes. There is no cost and participants are welcome to bring a support person.
How do you register your clients?
Working with GPs and other health professionals is an integral aspect of the DESMOND program pathway. With the client provided the opportunity to set their own diabetes management goals, this can inform team care arrangement planning, cycle of care review and referral to other services.

Patient who has been diagnosed with type 2 diabetes, be registered with the National Diabetes Services Scheme and be at least 18 years of age
